PHARMACY REFILLS & SUPPLEMENTS

Refills

Please have your pharmacy fax over a refill request- our fax number is 503-505-7672. Once your pharmacy faxes this over, please allow one week for that request to be filled.

Any requests that require more urgent attention will incur a fee of $30 which will need to be paid before the refill is made.

*Please note that legally I need to see you at least once per year to continue prescribing medications. It is best to schedule an appointment when you have filled your last refill so there is no delay.

Certain controlled substances require more frequent visits.

Supplements

We have a new pick-up box for zero contact supplement pick-up. Please make arrangements with Audra to have supplements left for you. Do not stop by the office without calling ahead.

Flat rate shipping for products not available through the online pharmacy or by request is $20 through our office. We needed to increase this charge due to rising shipping costs.
